Locate your Custom Field and check the box so the field can populate in your Web-Opt-In Page

Preview your Web-Opt-In Page to view your Custom Field

You should see your Custom Field populated in your Preview

Navigate to List Growth Tools, select Embedded Form, and click on Manage

Select Create New Embedded Form
