When using an embedded form on your website to collect customer information, there are specific disclosures that must be included to ensure that the customer knows what they are consenting to. This is required by TCPA/CTIA, so your customers know that, upon leaving their information with you, they are signing up to receive text messages from you.

If you are utilizing another service's embedded form, you will have to copy the required disclosures below and paste them into your embedded form:

I consent to receive SMS/MMS text messages from <COMPANY>. Click here for Terms&Conditions and Privacy Policy: <URL>. Recurring Messages. Msg&Data rates may apply. Reply HELP for help, STOP to opt-out.

Remember to replace <COMPANY> with your Company Name and <URL> with the link to your SMS Terms & Conditions and Privacy Policy.
